1. How did you hear about REACH? Who (if anyone) referred you? What current REACH families (if any) do you know? 2. Are you able to teach or assist in all three classes each Friday? (We do provide a nursery for non school age children during classes.) 3. Do you have a preference to what grades and which classes you are placed as a helper? (Please understand that the needs of the co-op will take precedence.) 4.

All REACH members are asked to be active contributors to our co-op, and we like to utilize each member's unique gifts. With that in mind, which committes do your skills/gifts best align with? (Please check any/all that apply.)

Enrichment Events -- helping with things that happen during regular REACH hours such as presentation day, field day, parties, etc.
Family Connections -- check in with families regularly, help with communication as needed, help coordinate assistance if any families have a need (ex: surgery, illness, new baby), etc.
Fundraising -- all ideas welcome!!
Kitchen Crew -- help with food and kitchen setup/cleanup for special REACH days and large events such as Santa Shop, formal, and graduation
Major Events -- help plan and execute major REACH extras such as Santa Shop, Formal, and Graduation

I understand that it is my responsibility to review any/all guidelines for students with my children prior to the start of each semester. I understand that if anyone in my family violates the rules, it may jeopardize our participation in REACH. I also understand that the Director reserves the right to deny participation to any student or family and the right to remove our family from REACH. Yes

7. I understand that in order to join REACH all adults who will teach/help at co-op must complete a background check. I understand that this background check will be run by First Baptist Church of Delmar through Ministry Safe on behalf of REACH and agree to submit the forms in a timely manner when asked. I understand that the cost of one background check per family is covered by REACH, but if additional background checks are needed I will be asked to pay an additional fee (typically $8 per background check). 8. By checking yes, I am agreeing to the following: I agree to be responsible for the behavior of the children in my charge. By submitting my non-refundable registration fee, I establish my family’s registration in REACH Christian Co-op, and I establish that I and my family are in agreement with all requirements, guidelines, and policies presently listed or as may subsequently be amended. 9. ASSUMPTION OF RISK. I acknowledge and understand the following:

1. Participation in co-op includes possible exposure to and illness from infectious diseases including but not limited to colds, flu, chicken pox, rotovirus, RSV, and COVID-19. I knowingly and freely assume all such risks related to illness and infectious diseases (such as COVID-19), even if arising from the negligence or fault of the Released Parties.

2. Participation in co-op includes the possibility of injury (sprains, breaks, etc). I hereby knowingly assume the risk of injury, harm and loss associated with the Activity, including any injury, harm and loss caused by the negligence, fault or conduct of any kind on the part of the Released Parties.

MEDICAL ACKNOWLEDGMENT AND RELEASE. I acknowledge the health risks associated with the Activity, including but not limited to illness, transient dizziness, lightheaded, fainting, nausea, muscle cramping, musculoskeletal injury, joint pains, sprains, strains, breaks, heart attack, stroke, or sudden death. I agree that if I experience any of these or any other symptoms during the Activity, I will discontinue my participation immediately and seek appropriate medical attention.

Furthermore, by checking yes, I agree that I will hold harmless and will not hold liable/responsible REACH Director, teachers, other members/children or the host church (First Baptist of Delmar) for any illness, injury, accident, damages, medical care/expenses, loss, etc. to myself, or my children. By checking yes, I further agree to pay for any item that my child(ren) damage or break in the church. Yes
